the egg. In the andrology laboratory, a sample with the highest possible count of quality sperm is obtained through processing of the partner&#8217;s or donor&#8217;s semen. An adequate dose of sperm is then injected through a thin catheter directly into the woman&#8217;s uterus. The procedure is similar to a conventional gynaecological examination. It is painless and quick.<\/p>\n<h5>For whom is the IUI treatment intended?<\/h5><p><span lang=\"en-GB\">Intrauterine insemination is a non-invasive method suitable for couples with slightly impaired spermogram, women with reduced cervical permeability, or women diagnosed with mild endometriosis.<\/span><\/p>\n<h5>Success rate<\/h5><p><span lang=\"en-GB\">The success rate of the IUI method is from 5-15\u00a0% per cycle (depending on the age of the woman). The course of the stimulation is monitored by the physician using ultrasound. Mature eggs are retrieved by puncturing the follicles under general anaesthesia with ultrasound guidance. On the day of egg retrieval, the partner&#8217;s semen is also collected. The retrieved eggs are fertilized with sperm and the resulting embryos are then stored (cultured) in the incubator. After several days of cultivation, the best quality embryo is transferred into the woman&#8217;s uterus \u2013 the so-called embryo transfer. The remaining high-quality embryos can be frozen and preserved for later treatment.<\/span><\/p>\n<h5>For whom is the IVF treatment intended?<\/h5><ul>\n<li>\n<p><span lang=\"en-GB\">Couples who have been trying to conceive for more than six months or a year and no cause of their infertility (unexplained sterility) has been found.<\/span><\/p>\n<\/li>\n<li>\n<p><span lang=\"en-GB\">Couples with a severe male sterility factor.<\/span><\/p>\n<\/li>\n<li>\n<p><span lang=\"en-GB\">Couples with an immunological factor of infertility.<\/span><\/p>\n<\/li>\n<li>\n<p><span lang=\"en-GB\">Women diagnosed with blocked fallopian tube.<\/span><\/p>\n<\/li>\n<li>\n<p><span lang=\"en-GB\">Women diagnosed with endometriosis.<\/span><\/p>\n<\/li>\n<\/ul>\n<h5>Alternatives to IVF method<\/h5><h6>IVF with minimal stimulation<\/h6>\n<p><span lang=\"en-GB\">This method produces fewer eggs than conventional IVF. In this type of stimulation, anti-estrogens or injectable preparations at a very low dose are used, so this treatment is gentler and safer for the woman.<br \/>\n<\/span><strong><span lang=\"en-GB\">In practice, minimal stimulation is mostly used in the following cases:<\/span><\/strong><\/p>\n<ul>\n<li>\n<p><span lang=\"en-GB\">Repeated failure of gonadotropin stimulation during IVF treatment.<\/span><\/p>\n<\/li>\n<li>\n<p><span lang=\"en-GB\">Previous stimulation was complicated by ovarian hyperstimulation syndrome.<\/span><\/p>\n<\/li>\n<\/ul>\n<h6><span><span lang=\"en-GB\">Native (natural) IVF cycle<\/span><\/span><\/h6>\n<p><span lang=\"en-GB\">When using a native IVF cycle, no hormonal stimulation of the ovaries is used. During the puncture, the goal is to retrieve at least one egg. This method has a relatively low success rate due to the use of only one egg. This protocol is used in women with a regular cycle and ovulation. The advantage is that almost zero hormonal preparations are used.<\/span><\/p>\n<h6><span lang=\"en-GB\">Transfer of thawed embryo after hormonal preparation of the uterus<\/span><\/h6>\n<p><span lang=\"en-GB\">For the preparation of uterine lining, it is necessary to use estrogens in the form of tablets, patches or spray according to the schedule prescribed by the physician. The goal is to prepare the optimal endometrial thickness of the uterus to be able to accept the implanted embryo. The quality of the uterine lining is checked by ultrasound between the 12th and 14th day of medication use and then the day of the addition of gestagens (and thus the exact day of transfer) is scheduled. This protocol is used in women with irregular periods or low endometrium.<\/span><\/p>\n<p><span lang=\"en-GB\">Both protocols have comparable success rates and it is up to the physician to decide which is more appropriate for the patient. The transfer is a painless procedure similar to a routine gynaecological examination and takes just a few minutes.<\/span><\/p>\n<h5>FET DEM \u2013 Frozen embryo transfer with donated embryo<\/h5><p><span lang=\"en-GB\">The above-mentioned procedures can also be used to prepare for the transfer of a donated embryo. The embryo comes from healthy young donors, examined in accordance with European tissue legislation and recommendations of professional societies. The selection of donors is based on the requirements of the treated couple in order to achieve the best possible match with their phenotype.<\/span><\/p>\n\r\n        <\/div>\r\n FERTILIZATION<\/h3>\r\n                <h4><\/h4>\r\n            <\/div>\r\n        <\/header>\r\n        <div class=\"accord-content\" style=\"display: none;\">\r\n            <h5>ICSI \u2013 Intracytoplasmic Sperm Injection<\/h5><p><span lang=\"en-GB\">In this micromanipulation method, a single sperm is injected directly into the egg. ICSI is performed utilising a special device \u2013 a micromanipulator. The success rate of fertilization is as high as 90\u00a0%.<\/span><\/p>\n<p><span lang=\"en-GB\">This method is suitable for couples with poor quality sperm or couples where a low number of eggs have been retrieved from the woman. In some couples (severe spermatogenesis disorder) the use of this method is unavoidable. The usage of the ICSI method, i.e., the selection of both sperm and egg, significantly increases the likelihood of producing higher quality embryos compared to conventional in vitro fertilization.<\/span><\/p>\n<h5>MACS \u2013 Magnetic-Activated Cell Sorting<\/h5><p><span lang=\"en-GB\">MACS <\/span><span lang=\"en-GB\">is a method of sperm processing with the use of specific antibodies on magnetic microspheres and magnets. This method enables the removal of antibody-labelled defective sperm cells from the semen and retention of only live undamaged sperm cells, which are later used for assisted reproductive techniques. The method is suitable for patients with poor quality sperm, patients with a low rate of egg fertilization after IVF with ICSI, or after repeated failure of IVF treatment cycles without apparent cause. <\/span><\/p>\n<h5>MFSS \u2013 Microfluidic Sperm Sorting<\/h5><p><span lang=\"en-GB\">The MFSS method is based on the principle of natural sperm selection by passage through micro-barriers imitating the natural environment of female reproductive system. In this method, sperm in the ejaculate are placed on a special chip and spontaneously pass through microchannels into the fertilization medium.<\/span><\/p>\n<p><span lang=\"en-GB\">MFSS is mainly performed in couples with embryonic arrest in previous cycles, with repeated abortions or embryo transfers without embryo implantation and in older women.<\/span><\/p>\n<h5>TESE\/MESA<\/h5><p><span lang=\"en-GB\">If azoospermia (absence of any sperm in the ejaculate) is repeatedly confirmed, in some cases it is possible to obtain sperm by surgical methods \u2013 MESA (Micro Epididymal Sperm Aspiration), in which sperm are microsurgically extracted from the epididymis, or TESE (Testicular Sperm Extraction), in which a small piece of tissue is surgically removed from the testicle. The material is subsequently processed and, if sperm are retrieved, they can be used for the ICSI method of fertilization.<\/span><\/p>\n\r\n        <\/div>\r\n    <\/section>\r\n<\/article>                                        <article class=\"accordion\">\r\n    <section>\r\n        <header>\r\n            <img src=\"https:\/\/stage.ivfclinic.cz\/wp-content\/uploads\/2022\/04\/icon-cool-icon-132-9.svg\" class=\"attachment-post-thumbnail size-post-thumbnail wp-post-image\" alt=\"\" decoding=\"async\" loading=\"lazy\" \/>\r\n            <div class=\"header-group\">\r\n                <h3>PGT<\/h3>\r\n                <h4><\/h4>\r\n            <\/div>\r\n        <\/header>\r\n        <div class=\"accord-content\" style=\"display: none;\">\r\n            <h5><\/h5><p><span lang=\"en-GB\">Within the framework of assisted reproduction methods, it is possible to perform genetic testing and selection of embryos before transfer to the uterus and implantation (preimplantation testing). The method of examination is based on the conclusions of the genetic consultation. Preimplantation testing can either be targeted at diseases already present in the family or at random genetic changes resulting from the external environment or the age of the parents. A cell or several cells are separated from the embryo and then sent to our affiliated genetic laboratory for testing.<\/span><\/p>\n<h6><span lang=\"en-GB\">There are two basic types of preimplantation testing:<\/span><\/h6>\n<h5>PGT<\/h5><p><span lang=\"en-GB\">PGT\u00a0(preimplantation genetic testing) is a <\/span><span lang=\"en-GB\">targeted testing of embryos to exclude hereditary familial diseases<\/span> <span lang=\"en-GB\">where one or both partners are carriers of disposition for specific disease. This may be a chromosomal defect (PGT-SR) or a monogenic hereditary disease (PGT-M) such as cystic fibrosis, spinal muscular atrophy or haemophilia.<\/span><\/p>\n<p><span lang=\"en-GB\">In the case of sex-linked diseases (e. g. haemophilia), it is also possible to make a selection of embryos of the sex which does not have the risk of the disease. Otherwise, selection of embryos according to the sex is legally not possible. Regarding the tracked diseases, only healthy embryos are transferred into the uterus, thus minimising the risk of a having an unhealthy child or having to terminate the pregnancy when foetal impairment is detected.<\/span><\/p>\n<h5>PGT-A<\/h5><p><span lang=\"en-GB\">PGT-A<\/span><span lang=\"en-GB\"> (preimplantation genetic testing for aneuploidy)<\/span><span lang=\"en-GB\"> is a method of testing embryos for chromosomal abnormalities before transfer to the uterus. Many IVF cycles can end in disappointment due to the failure of the embryo to implant (nest). One cause is an incorrect amount of genetic material (chromosomes) in the transferred embryo. The loss of a chromosome or an extra chromosome in the embryo is referred to as aneuploidy.
